MSW LMSW SW Care Coordinator - Complex Case Assignment
UT Southwestern Medical Center, a world‑renowned medical and research center in Dallas‑Fort Worth, Texas, seeks an LMSW Care Coordinator with a specialized assignment as a Complex Case Manager. The role focuses on high‑risk and complex psychosocial/medical patients at Clements University Hospital and Zale Lipshy Pavilion, managing discharge planning and serving as a primary care coordinator.
Job Summary
This position supervises the care coordination process for complex inpatient cases, providing education to the healthcare team about psychosocial issues, consulting across departments, and ensuring patient safety during transitions.
Skills Required
Excellent written and verbal communication
Superior knowledge of community resources
Ability to educate patients on Medicaid, Medicare, insurance plans and limitations
Verbal de‑escalation techniques
Superior interdepartmental coordination
Excellent knowledge of medical social work tasks and inpatient workflows
Proposed Schedule
Monday – Friday, 8:00 am – 4:30 pm

Experience and Education
Licensed LMSW, Master’s in Social Work, Texas license
Preferred
Minimum 2 years of hospital experience
Job Duties
Screen high‑risk patients for discharge planning needs; consult physicians and interdisciplinary team on psychosocial barriers.
Collaborate with the multidisciplinary team to implement and adjust transition plans.
Facilitate patient care conferences, especially for complex cases, to establish safe discharge plans.
Provide referrals to community resources and convey options to patients.
Assist with abuse, neglect, and adoption cases, providing investigation, intervention, and referrals.
Educate patients and families on community resources, benefits, and limitations.
Maintain communication with community agencies for referrals.
Stay current on payer benefits, hospital resources, regulatory standards for informed decision‑making.
Maintain clinical notes, case histories, and statistical data as required.
Perform related duties as assigned.
