Overview The social worker Care Coordinator is a member of the Care Coordination Department who educates the healthcare team and physicians about psychosocial issues and patient/family problems, and applies assessment, planning, implementation and evaluation, case management, counseling, information and referral, education, research, advocacy, and program administration. This role integrates national standards for care coordination and transition management to support successful transitions of care.
Responsibilities Screens and evaluates high-risk patients for discharge planning needs. Consults with attending physicians and the healthcare team regarding psychosocial issues and care transition barriers.
Collaborates with the multidisciplinary team, patient, and family to execute plans and interventions that facilitate the hospital stay and manage length of stay; reassesses as care needs change.
Facilitates patient care conferences to establish safe and effective discharge plans.
Implements the transition of care plan through appropriate referrals and ensures patient choice in agencies and services.
Assists with cases involving adoption, abuse and neglect, including assessment, investigation, intervention, and referral to appropriate agencies as indicated.
Educates patients and families about community resources and interprets needs and limitations of resources.
Maintains open communications with community agencies to assist in referring and meeting patient needs.
Maintains knowledge of payor benefits, hospital and community resources, and regulatory standards to ensure informed decision making and desired outcomes.
Maintains chronological notes, clinical charts, statistics, and case histories for each patient regarding social issues and interventions.
Performs other duties as assigned; duties may involve direct patient care, care of research participants, or handling patient records as applicable.
Qualifications Required
Licenses and Certifications: Licensed Master Social Worker (LMSW) in the state of Texas.
Preferred
Experience: 2 years hospital experience.
