Job Summary
NCSS is seeking a dedicated Care Coordinator to join our team and provide integrated, person-centered support to military members, veterans, and their families.

Responsibilities

Ensure individuals receive timely, coordinated care that reflects their goals and needs.

Collaborate across systems with healthcare providers, community agencies, and the Department of Veterans Affairs to help clients access the right level of care, navigate benefits, and transition smoothly between services.

Support ongoing case management efforts, assist with administrative and documentation processes, and participate in quality improvement initiatives.

Maintain close communication with clients and partners to promote engagement and wellness.

Focus on empowering veterans and service members by addressing unique needs such as trauma recovery, reintegration challenges, housing stability, and behavioral health coordination.

Provide flexible, strong communication and culturally competent, trauma-informed care.

Qualifications

Bachelor’s Degree preferred or equivalent combination of related education, training and experience.

Experience working with or within the VA, Department of Defense, or veteran-serving nonprofits is highly valued.

Strong communication skills and commitment to trauma-informed, culturally competent care.
