Schedule: Monday - Friday, 10 AM - 6 PM

Position Memory Care Coordinator
Responsibilities
Under the supervision of the Director of Nursing and the Executive Director, ensure high quality resident care within the memory care unit.
Manage memory unit staff, including training, development, evaluation, scheduling, and personnel problem solving.
Develop and facilitate programming to meet the specific needs of residents with Alzheimer’s disease and dementia.
Assist with admissions of new residents and ensure provision of appropriate care by reviewing care plans daily.
Understand and follow the facility's Healthcare Clinical Policies and Procedures and Safety Guidelines.
Promote independence for residents and develop/implement activities programs based on assessments and resident interests.
Conduct resident screenings and family interviews to assess program needs; write monthly activities calendars with input from Activities Staff.
Oversee facility activity programs and ensure documentation is completed in a timely fashion.
Manage care staff in the memory unit, including hiring, training, scheduling, and evaluating; perform general office functions to support the department.
Resolve resident complaints in collaboration with management and staff; assist in the absence of the Director of Nursing as needed.
Order, store, and maintain supplies and equipment within budget; ensure cleanliness and maintenance of the community and memory care unit.
Comply with health care and safety standards; assist families and visitors as needed; attend required in-services and meetings.
Perform other duties as directed.
Qualifications
High School Diploma or equivalent required. Associate's or Bachelor's Degree preferred.
Basic knowledge of Alzheimer's disease and related dementia.
2 to 3 years of gerontological and caregiving experience preferred.
