Position Summary
The Medical Case Manager will provide information to clients and their families, answer factual questions and give explanations and interpretations of organizational policies and procedures. This position provides medical case management, referrals, eligibility determination and crisis intervention services to adults with HIV infection receiving medical services in the outpatient Infectious Diseases Clinic. The employee will complete assessments, identify service needs, develop care plans, facilitate linkage to services in the clinic, at UNC Health, or in the community. They may assist with transportation and housing needs. Responsibilities may include assisting clients in accessing financial benefit programs. The employee will work closely with the existing licensed social work team, medical providers, nursing staff and benefits coordinators as part of an interdisciplinary team. Requires timely documentation and data entry in an electronic medical record (EPIC), electronic databases and tracking systems. Successful employees possess a strong ability to multi-task in a fast-paced environment. Employees are required to attend trainings and meetings as directed.
Minimum Education And Experience Requirements
Bachelor’s degree in a Human Services discipline with coursework related to the area of assignment.
Some positions may require certification to practice as a certified or licensed Clinical Social Worker as required by the North Carolina Social Work Certification Board.

The discipline of Human Services requires coursework and experience pertinent to the specific area of work.
Preferred Qualifications, Competencies, and Experience
Certification in case management is preferred.
Experience with clinic population, electronic medical records, case management, and data management preferred.
