Responsibilities

Provide case management services for 50 HIV positive individuals.

Provide emergent case management services for EIP patients as needed.

Complete case management assessments and develop client service care plans.

Monitor and evaluate service care plans quarterly.

Assess all EIP clients requesting or referred for CM services, identify immediate/urgent needs, and take appropriate action.

Meet objectives for the provision of referrals and linkages to support services.

Provide HIV counseling and testing as needed.

Ensure CM services comply with AACO and NJDHSS standards for HIV medical case management services.

Experience Required

1 year or more experience in case management or social services.

Experience in the field of HIV preferred.

Extensive knowledge of available financial and social service resources in southern New Jersey.

Excellent interpersonal skills.

Education Requirements

Bachelor's Degree required.

License/Certification Requirements
AACO or NJ HIV Case Management Certification preferred.

Special Requirements
Bilingual is a plus.
