Job Description
The Medical Assistant patient Care Coordinator professional is to serve as a liaison between health care providers and patients. Duties include triaging patient phone calls, documentation of all actions, communications and assisting with clinic visits as needed. Supports and promotes the vision, mission, and strategic plans of Methodist Health System.

Job Requirements

High school Diploma or Equivalent required

Successful completion of Medical Assistant Program required

1 year experience in direct patient contact in a medical setting preferred

Proficient with technical office equipment including computers, printers, fax, copiers and telephones

Strong proficiency using Microsoft Office products

Strong oral and written communication skills

Bilingual skills preferred

Uses good problem solving skills

Strong process orientation

Detail oriented, logical, and methodological approach to problem solving

Ability to provide vision and leadership

Ability to plan and schedule the work of others

Demonstrated capability to interface and maintain effective relationships with all departments and employees in a team‑oriented environment

Assess daily work load to determine volume and urgency; prioritize and adapt to changing patient, physician and professional allied health provider workflow

Enter appropriate, evaluation, pre‑ and post‑transplant data, test/procedure results obtained from external and internal sources into transplant database

Obtain lab and other clinical data required maintaining patients' appropriate MELD score

Monitor pre‑ and post‑transplant patient clinical data and assure data is reviewed by a physician as required

Obtain and communicate test results to patients as directed

Communicate patient concerns to appropriate health care provider

Mail appropriate correspondence to all referral sites, referring physicians and patients in a timely manner

Notify health care provider of test results in a timely manner
