Job Title: My Community Cares (MCC) Case Manager

Job Purpose: Partners directly with families to help reduce the likelihood of initial or repeat involvement with the Department of Children and Family Services (DCFS). Provides short-term case management for families needing additional support to identify needs and access available resources.

Experience and Knowledge

Bachelor’s degree in social work or related field

Minimum of 2 years direct service, including case management required

Knowledge of community, job, financial, and social resources

Trust-Based Relational Intervention and Adverse Childhood Experiences (ACES) training preferred

Demonstrates ability to partner with families using a strengths-based approach

Preference will be given to candidates who most reflect the racial, ethnic, and socioeconomic status of the majority of members of the neighborhoods being served

Assessment, intake or interview experience required

Must have reliable transportation, valid driver’s license, and meet state required automobile insurance minimums

Skills And Abilities

Ability to communicate effectively with internal and external customers on the telephone and in person in a professional, tactful, and friendly manner

Excellent verbal and written communication skills, including the ability to record with clarity and promptness

Proficiency in Microsoft Office, including Word, Outlook, Excel and Power Point

Ability to develop and maintain professional relationships with individuals from various cultural, ethnic and racial backgrounds

Effective in planning and arranging work responsibilities including determining priorities, managing multiple projects, handling deadlines and changes in priorities

Knowledge of current social and economic problems and the way these problems affect families and individuals

Ability to communicate with and be sensitive to the needs of people of various cultural, ethnic and racial backgrounds

Ability to access client’s financial and social needs and to determine priorities

Ability to assist a person with budget preparation, resumes, interview skills, and soft skills for maintaining a job

Ability to identify and use community job placement/financial resources

Maintains poise and self-control even under extreme stress and unexpected crises; handles delicate situations tactfully

Job Related Training

Orientation

Initial 40 hours Domestic Violence and Sexual Assault as required by Wellspring

Initial and ongoing MCC training in the areas of protective factors that promote child wellbeing, individual and community level trauma and its impact, best practices in preventing child abuse and neglect, and disparities and disproportionalities in public systems

Trust-Based Relational Intervention and Adverse Childhood Experiences (ACES) training

40 hours continuing education yearly

Ongoing in-service workshops, conferences and self-study

Monthly Agency Staff meetings, program delivery meetings and program area meetings

Principle Duties and Responsibilities

Delivers services by providing short-term case management for families needing support in identifying needs and accessing available resources. Case management is provided using either the Solution Based Casework model or through another well-defined, evidence-based model already in use at The Wellspring.

Conducts initial intake and ongoing assessments with families. Gathers, tracks, and reports related data using systems and processes in place at The Wellspring.

Connects individuals to the services and supports in their community, which includes evaluating needs, addressing accessibility, making and tracking referrals, partnering with agencies and community members, advocating to overcome barriers, and providing short-term case management and follow-up services.

Tracks and reports required qualitative and quantitative data to the MCC Parish Director and MCC State staff.

Reports to the MCC Program Coordinator and utilizes staff for support in implementation of responsibilities.

Assists clients with exploring options and setting financial and personal goals.

Assists clients with preparing resumes, interview skills, budget planning, and soft skills to maintain a job.

Obtains assistance for the client by referring to appropriate community resources, arranging appointments, and establishing rapport with other agencies.

Develops contacts with local businesses to identify job opportunities for clients.

Maintains a positive attitude and demonstrates concern and respect for others by listening carefully and without judgment.

Establishes personal rapport with clients and maintains case records by documenting client activity and actions.

Monitors progress of client goals and provides regular follow-up.

Maintains operations by following policies and procedures; participates in quality reviews and reports needed changes.

Ensures quality services and program efficiency by complying with program standards and guidelines and completing required agency and funder paperwork in a timely and accurate manner.

Maintains professional and technical competency by continued education, reviewing professional publications, and establishing networks.

Maintains safety of clients and staff by complying with agency procedures, following safety procedures, and maintaining confidentiality policies and procedures.

Contributes to team effort by communicating effectively, helping others, showing flexibility, and engaging in cooperative contacts.

Performs other duties as related to the position and may work non-traditional hours for special projects or deadlines.
