Madison County Moderate Care Coordinator
National Youth Advocate Program (NYAP) is seeking a full‑time Moderate Care Coordinator to serve children and youth enrolled in the OhioRISE program across Madison County. The role supports families in navigating Ohio Medicaid’s OhioRise program and delivering community‑based, wrap‑around care coordination.
Compensation: $45,000 per year.
About OhioRISE
OhioRISE (Resilience through Integrated Systems and Excellence) is Ohio’s first highly integrated care program for youth with complex behavioral health and multi‑system needs. NYAP was selected as the Care Management Entity for Catchment Area C, covering Madison County (and overlapping Clark County as needed).
Responsibilities
Cultivate flexible, family‑focused, community‑based responsive services based on the high‑fidelity wrap‑around model of care coordination.
Develop and maintain the wrap‑around team, including coordinating and leading team meetings.
Coordinate and supervise implementation of the Plan of Care, including a transition plan and crisis plan with providers and community resources; update plan as necessary.
Ensure family support and stabilization during crises.
Provide and document the initial and ongoing life domain assessment.
Maintain all service documentation requirements, evaluation outcome requirements, and data as required.
Provide services in a timely manner in accordance with Plan of Care and/or Crisis Plan.
Utilize and monitor flexible funding and service coordination.
Obtain weekly reports from subcontracted providers.
Participate in after‑hours on‑call response.
Attend program staff meetings, supervision, and any other meetings as required.
Participate in the agency and program CQI peer‑review process.
Perform duties to reflect agency policies and procedures and comply with regulatory standards.
Meet agency training requirements.
Report all M&U’s to site manager and supervisor immediately.
Other duties as assigned.
Qualifications
Licensed or unlicensed practitioner in accordance with rule 5160‑27‑01 of the Administrative Code.
Completion of the high‑fidelity wrap‑around training program provided by an independent validation entity recognized by ODM.
Successful completion of skill and competency‑based training to provide MCC.
Experience providing community‑based services to children and youth and their families in behavioral health, child welfare, intellectual and developmental disabilities, juvenile justice, or a related public sector human services or behavioral health care field.
Educational background: either
Three years with a high school diploma or equivalent.
Two years with an associate’s degree or bachelor’s degree.
One year with a master’s degree or higher.
Background and experience in one or more: family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development.
Culturally competent or responsive with training and experience necessary to manage complex cases.
Experience working with children and families experiencing SED, trauma, co‑occurring behavioral health disorders and who are engaged with one or more child‑serving systems.
Residence in one of the counties included in Catchment Area C.
Driving and Vehicle Requirements
Valid driver’s license.
Reliable personal transportation.
Good driving record.
Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability.
