Position Summary
The Clinical Care Coordinator is a Licensed Vocational Nurse who engages in community outreach to counsel, explain, and educate patients and families about hospice and palliative care services. Responsibilities include reviewing consents and election of hospice benefit, coordinating patient discharge from hospital into hospice or palliative care programs, and performing start‑of‑care and initial nursing assessment. The Coordinator works cooperatively with hospice and/or palliative care teams, meets with patients/families in person or remotely, and utilizes electronic medical records.

Qualifications

Graduate of an accredited nursing school with a current California nursing license.

Certification in Hospice and Palliative Nursing encouraged.

At least one year of nursing experience within the last 3 years.

Excellent verbal and written communication skills.

Spanish speaking a plus.

Valid driver’s license, reliable transportation, and auto insurance.
