LTSS Service Care Manager - Behavioral Health
Service Delivery Area: Waco, TX

Monday – Friday, 8 AM – 5 PM (CST); no evenings, weekends, or on‑call work.

Travel required: 3 days per week.

Position Purpose
Develops, assesses, and coordinates holistic care management activities, with primary focus and support toward populations with significant mental/behavioral health needs, enabling quality, cost‑effective healthcare outcomes. Evaluates member service needs, develops care plans, and educates members, families, and caregivers on available services and benefits.

Responsibilities

Evaluate the needs of complex high‑risk members with mental/behavioral health challenges and recommend a plan of care for optimal outcomes.

Act as liaison and advocate between member/family, physician, and facilities/agencies.

Support members with conditions such as major depression, bipolar disorders, schizophrenia, borderline personality disorder, PTSD, substance use disorder, self‑injurious behavior, and psychiatric inpatient admissions.

Perform frequent home and/or site visits (once a month or more) to assess member needs and collaborate with resources.

Provide and facilitate education to members and families on preventive care, treatment options, medication regimens, and healthcare benefits. Offer expertise on crisis call handling, mental health first aid, field safety, de‑escalation, psychotropic monitoring, etc.

Coordinate community resources—including medical, behavioral, and social services—and manage service authorizations (e.g., meals, employment, housing, foster care, transportation).

Ensure appropriate referrals and make provider and community resources accessible to members.

Maintain accurate documentation in the electronic care management system, ensuring compliance with clinical, state, and federal guidelines.

Provide feedback to leadership on opportunities to improve quality of care and service delivery.

Perform other duties as assigned and comply with all policies and standards.

Education / Experience
Requires a Master’s degree in Mental Health or Social Work, or a Graduate from an Accredited School of Nursing, plus 2–4 years of related experience.

License / Certification

Licensed Behavioral Health Professional or RN based on state contract requirements (e.g., LCSW, LPC, or RN with Behavioral Health experience) is required.

Preferred Experience

3+ years of case management, care coordination, or discharge planning with adult populations (ages 18–65).

2+ years coordinating and managing healthcare/behavioral health services and providing patient advocacy and education to Medicaid members.

Experience in a field‑based Social Worker or Case Manager role in inpatient behavioral health hospital, community health, outpatient mental health, substance abuse/detox recovery treatment, or state social services settings.

Pay Range: $26.50 – $47.59 per hour.
