Position Overview
Centene is hiring – LTSS Service Care Manager (Hybrid, Lake County, FL)

This hybrid role covers Lake County, including Clermont, Groveland, Tavares, Eustis within zip codes 34711, 34736, 32778, 32726.

This position is 75% field-based, with the opportunity to work remotely when not visiting members. Standard work hours are Monday – Friday, 8:00 AM – 5:00 PM (local time).

Key Responsibilities

Provide case management support for members in their homes or facilities

Collaborate with healthcare providers, social service agencies, and community resources

Ensure members receive high-quality, person-centered care

Position Purpose
Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with personalized service care plans for long-term care members and educate members and their families/caregivers on services and benefits available to meet member needs.

Evaluates member needs, available resources, and recommends or facilitates optimal plans

Develops ongoing long-term care plans and identifies necessary providers or community resources

Coordinates between members, families/caregivers, and provider teams to ensure services are accessible

Provides resource support for employment, housing, participant direction, independent living, justice, foster care based on assessments and plans

Monitors care plans, member status, and outcomes, providing recommendations as needed

Collaborates with healthcare providers and partners to meet member needs

Collects, documents, and maintains member information and care management activities for regulatory compliance

Performs home and site visits to assess member needs and collaborate with providers

Educates members and caregivers on procedures, provider instructions, service options, referrals, and benefits

Provides leadership with feedback to improve quality of care and service delivery cost-effectively

Performs other duties as assigned

Education & Experience
Requires a Bachelor’s degree and 2–4 years of related experience, or equivalent experience reflected in accomplishments.

Compensation
Pay Range: $26.50 - $47.59 per hour
