Overview
Centene is transforming the health of our communities. This role is the first line of support for our 28 million members. The position requires 80% field work, visiting members in their homes or care facilities in the Greenville area.
Responsibilities
Evaluates the needs of the member and recommends a plan for the best outcome.
Develops and maintains long‑term care plans, identifies providers and community resources.
Coordinates services between members, families/caregivers and the care provider team.
Provides resource support for employment, housing, independent living, justice, foster care based on assessment and plans.
Monitors care plans and member status, recommends adjustments based on identified needs.
Interacts with long‑term care healthcare providers and partners to ensure member needs are met.
Collects, documents, and maintains member information and care management activities to ensure compliance with regulators.
Performs home and site visits to assess member needs and collaborate with healthcare providers.
Facilitates education to members and families/caregivers on procedures, provider instructions, service options, referrals, and benefits.
Provides feedback to leadership on opportunities to improve quality of care and service delivery.
Performs other duties as assigned and complies with all policies and standards.
Qualifications
Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW or LCSW‑A) with experience in physical health, case management, or field‑based care.
At least 2–4 years of related experience, or equivalent experience through applicable knowledge, duties, scope and skill reflective of this level.
For North Carolina, tailor plan two years of prior LTSS and/or HCBS coordination, care delivery monitoring and care management experience; prior experience with social work, geriatrics, gerontology, pediatrics, or human services.
RN or LCSW / LCSW‑A preferred.
Education/Experience
Requires a bachelor's degree and 2–4 years of related experience, or equivalent experience.
License/Certification
For North Carolina: two years of prior LTSS and/or HCBS coordination experience.
Pay Range
$26.50 – $47.59 per hour.
