2 Positions Available
Applicants must reside in one of the listed ZIP codes and be able to travel within these ZIP codes to visit assigned members in the community for physically disabled & elderly population. Zip codes: 60632, 60609, 60638, 60629, 60636, 60621, 60652, 60620, 60655, 60643, 60406, 60482, 60803, 60805, 60653, 60615, 60637, 60649, 60619, 60617, 60628, 60827, 60633, 60616.

Role will work from home with 75% travel in listed zip code areas. Mileage is reimbursed at the current IRS guideline rate round trip.

Position Purpose
Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans for long‑term care members and educate members and their families/caregivers on services and benefits available to meet member needs.

Responsibilities

Evaluate the needs of the member, the resources available, and recommend and/or facilitate the plan for the best outcome.

Assist with developing ongoing long‑term care plans and work to identify providers, specialists, and community resources needed.

Coordinate as appropriate between the member, family/caregivers, and the care provider team to ensure identified services are accessible to members.

Provide resource support to members and families/caregivers for various needs (employment, housing, participant direction, independent living, justice, foster care) based on service assessment and plans.

Monitor care plans, member status and outcomes and provide recommendations based on identified member needs.

Interact with long‑term care healthcare providers and partners as appropriate to ensure member needs are met.

Collect, document, and maintain long‑term care member information and care management activities to ensure compliance with state, federal, and third‑party payer regulators.

May perform on‑site visits to assess member’s needs and collaborate with providers or resources as appropriate.

Provide and/or facilitate education to long‑term care members and their families/caregivers on procedures, provider instructions, service options, referrals, and benefits.

Provide feedback to leadership on opportunities to improve and enhance quality of care and service delivery for long‑term care members cost‑effectively.

Perform other duties as assigned.

Comply with all policies and standards.

Education & Experience
Requires a bachelor’s degree and 1 year of related experience or equivalent experience attained through coursework and duties reflective of the position.

Illinois Plan Only
In addition to the above requirements, employees working with physically disabled/elderly population must meet one of the following criteria:

RN licensed in Illinois.

Bachelor’s or Master’s degree prepared in human services related field.

LPN with one year experience in conducting comprehensive assessments and providing formal services for the elderly.

Pay
Pay Range: $22.50 – $38.02 per hour.
