Position Overview
This position is primarily a work‑from‑home role with expectations to attend in‑home visits as needed and in‑person meetings quarterly. The focus includes medical records management, protocol development, compliance review, and communication with nursing and other functional teams.
Duties & Responsibilities
Organize, retrieve, review, and file medical records, including the creation and maintenance of Medication Administration Records.
Serve as a liaison between the medical care community—including primary care providers, emergency services, and specialists—to gather necessary information for individuals receiving services.
Develop effective working relations and cooperate across teams throughout the entire case‑management process.
Teach and train providers, families, and staff in:
Medication Administration (QMAP) and MRB training.
Individual‑specific medical documentation.
Evaluate signs of illness or dysfunction that warrant medical or nursing intervention.
Basic skills required to meet the health needs of the individual.
Utilizing specialized medical equipment.
Create tracking for diet and home exercise plans as prescribed by the prescribing physician for individuals in the caseload.
Assist RN in the development, writing, and training of person‑specific medical protocols.
Review incoming documentation on new admissions to identify needs and establish medical files.
Perform basic office duties such as emailing, faxing, printing, copying, scanning, and inventory management.
Maintain accepted standards of confidentiality and HIPAA compliance.
Understand and meet a code of ethics and abide by the Standard of Practice relevant to the medical profession.
Two years of professional healthcare experience as an RN or LPN, preferably with persons with intellectual or developmental disabilities, providing knowledge of physical conditions and treatment protocols.
Preferred Experience
Experience working within the IDD population.
Case‑management experience.
Required Education, License or Certification
LPN license in good standing in the state of Colorado.
Physical Demands
The position may require certain physical activities in the office, at home, or at various job sites, including but not limited to: ascending/descending ladders or stairs; sedentary work that involves sitting, standing, bending, reaching, grasping, and/or kneeling; light work moving/lifting objects up to 20 pounds; repeated motions involving the wrists, hands, and/or fingers; commuting/driving to various locations.
