Base: $24.20/hr - $32.30/hr (Actual pay based on skills and experience)
Job Overview
The LPN Medication Care Manager is responsible for providing high‑quality care by administering medication and treatments in a safe, organized manner. The role includes medication administration, documentation, risk management, resident care, and team collaboration, all aligned with Sunrise Senior Living’s mission to champion the quality of life for all seniors.
Key Responsibilities
Administer medications and treatments safely, following the Six Rights of Medication.
Document all medication administration in the electronic health record accurately.
Maintain medication carts and medication rooms, ensuring cleanliness, availability, and expiration control.
Perform medication counts, including narcotics, with supervisors and report discrepancies.
Assist with medication reviews, resident care plans, and individual service plans.
Communicate with Resident Care Directors (RCD) or Wellness Nurses regarding medication status, concerns, and unavailability.
Support resident independence and dignity while adhering to Sunrise’s Principles of Service.
Participate in daily cross‑over meetings, risk‑management activities, and safety trainings.
Maintain compliance with infection control, occupational safety, and regulatory requirements.
Act as a team member, contributing to engagement programs and training.
Qualifications
LPN, LVN, or state/province‑specific licensed nurse credential.
Valid CPR and First Aid certifications.
Minimum of 18 years old.
Experience working with seniors preferred.
Desire to serve and care for seniors.
Drivers who use the company vehicle must sign the Driver job description.
Must maintain all required licenses, certifications, and ongoing training.
