Job Overview
The LPN Medication Care Manager is responsible for providing the highest quality care by administering medication and treatments in a safe, organized manner. Responsibilities include, but are not limited to, administration of medications, documentation of medication administration, conducting SHUs, and providing resident care while demonstrating the Sunrise Mission, “to champion the quality of life for all seniors,” in accordance with laws, standards, and Sunrise policies to promote the highest quality care for residents.

Medication Administration
Provide the highest quality care by administering medication and treatments in a safe, organized manner.
Review, read, notate, and initial the electronic health record to document and learn about residents.
Receive medication updates from the Resident Care Director (RCD) or Wellness Nurse.
Administer, assist with, and observe medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass.
Ensure medications are passed according to times using a mobile medication cart.
Document and initial as medications are given and document refusals or missed doses.
Maintain confidentiality of all resident information including medications.
Report resident concerns or unavailable medications to the RCD or Wellness Nurse.
Restock medication cart after all passes.
Assist in checking medication regardless of packaging system.
Assess residents for “as needed” medications and document/report to supervisor.
Count narcotics with another MCM or Lead Care Manager each shift and report discrepancies to the RCD or Wellness Nurse.
Maintain and clean the Medication Room, carts, and treatment carts; manage inventory and expired meds.
Follow the refill process for medications.
Help residents maintain independence and promote dignity and safety in accordance with Sunrise Principles of Service.
Respond empathetically to each resident, mindful of communication patterns, history, and needs.
Practice proper standard care precautions, cleanliness, hygiene, and health.
Audit medication carts.
Resident Care
Notify RCD of resident and/or family concerns.
Attend and participate in daily Cross Over meetings facilitated by the LCM.
Conduct Service and Health Updates as directed by RCD.
Participate in the development of the Individualized Service Plans (ISP).
Transcribe orders.
Risk Management and General Safety
Partner with the community team to ensure compliance with regulations concerning occupational health and safety, risk management, and safety policies.
Practice safety procedures, including PPE, fire safety, SDS, and lockout/tagout.
Report accidents/incidents and unsafe conditions immediately.
Ensure cords, carts, equipment, and hazards do not block exits and comply with fire codes.
Comply with infection control techniques and biohazard disposal policies.
Understand and practice handling of bloodborne pathogens, infectious waste, sharps, and hazardous materials; report exposures immediately.
Ensure oxygen tanks are stored safely and monitored per prescribed levels.
Training and Team Contribution
Participate as a team member and work toward shared goals.
Model the Team Member Credo in daily interactions.
Contribute to resident and guest engagement programs and improvement planning.
Attend regular meetings and trainings; may be designated shift supervisor and may supervise other MCMs.
Maintain compliance with required training and Sunrise standards; perform other duties as assigned.

Experience and Qualifications
To perform the job successfully, individuals must be able to perform each essential duty satisfactorily. Requirements are representative of skills and abilities needed.
LPN, LVN, or state/province-specific licensed nurse credential
Maintain certifications as required by state/provincial regulations
CPR and First Aid certifications
Must be at least 18 years of age
Previous experience working with seniors preferred
Desire to serve and care for seniors
If applicable, driver for Sunrise vehicles must sign Driver job description and comply with safety duties
