Location: Hollywood, FL (In person 5 days per week; 9-5:30)
Schedule: Full-Time
Salary Range: $64,000 — $70,000 per year

Position Overview Our client is seeking a dedicated LPN Clinical Care Coordinator to support care management efforts with a focus on both chronic care and behavioral health needs. In this role, you will review patient charts, support care planning, and facilitate communication between providers and clinical teams to ensure the highest standard of patient outcomes.
Key Responsibilities Provide assessment and care management services.
Administer validated rating scales to monitor patient progress.
Initiate and revise behavioral health care plans as needed.
Deliver brief psychosocial interventions when appropriate.
Collaborate closely with billing practitioners and clinical providers.
Maintain accurate registry and tracking sheets.
Consult with psychiatric specialists to inform care decisions.
Build and maintain continuous relationships with patients.
Foster integrated collaboration with the interdisciplinary care team.
Lead care plan meetings to review and update patient cases.
Qualifications Current Licensed Practical Nurse (LPN) degree/certificate required.
Prior experience in long-term care preferred.
Behavioral health experience strongly preferred.
Strong communication and care coordination skills.
