Job Summary
Provide ongoing support and knowledge through comprehensive assessment, planning, and implementation and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integration and functions of case management and utilization review. The person in this position is self‑directed, has a positive attitude, has the ability to motivate others toward achieving goals, and has a strong sense of and respect for confidentiality involving both patients and fellow employees.
Requirements
LPN with previous case management experience.
Effective communication – both written and verbal.
Excellent interpersonal skills, ability to establish rapport.
Knowledge of clinic policies/procedures, practices, techniques, and equipment as acquired through orientation.
Keyboarding skills required.
Windows skills preferred.
Physical Demands
Ability to lift up to 100 lbs, carry up to 40 lbs, push/pull up to 350 lbs.
Frequent sitting, standing, walking, bending, stooping, and reaching.
Close eye work.
Hearing normal and soft tones.
Distinguish smells & temperatures by touch and proximity.
Exposure to biological hazards, frequent unpleasant odors.
