Job Description Summary
This position is a work‑from‑home, full‑time, day‑shift role. No weekends and no holidays. Hours are 8 am – 4:30 pm. Must be experienced LPN and flexible to ever‑changing tasks the role may bring. Responsibilities include identifying care gaps, following up from HAI calls, assisting with disease management, scheduling appointments, and working through work queues. The LPN provides nursing care to patients and families along the health‑illness continuum in diverse health care settings while collaborating with the health care team. The LPN is accountable for the practice of nursing as defined by the Ohio Board of Nursing and delegates nursing tasks to UAPs using the criteria in the Ohio Nurse Practice Act. The position serves as a resource for other associates, may communicate instructions, do minor scheduling, and inspect work. The job has no supervisory responsibilities.

Responsibilities and Duties

25% Perform initial, ongoing, and functional health status assessment as applicable to the population and/or individual. Collect and document initial and ongoing subjective and objective data from patients, significant others, and communities. Review data obtained by other members of the health care team and incorporate into assessment. Identify and communicate assessment findings, both initial and ongoing, to appropriate people via referrals, rounds, huddles, reports, and care conferences. Use assessment data to identify pertinent nursing diagnoses and collaborative problems. Collaborate assessment findings with providers and members of the healthcare team as appropriate. Use scientific knowledge, critical‑thinking skills, technical skills, and effective interpersonal and teaching skills to provide nursing care.

25% Based on nursing diagnoses and collaborative problems, document planned nursing interventions to achieve outcomes appropriate to patient needs including age, size, and development stage. Initiate and/or coordinate care plans and individualize critical paths/medical management plans. Promote cost‑effective care by minimizing fragmentation, maximizing coordination, and facilitating patient/family through the health care system. Accountable for quality improvement and clinical outcomes and any other duties assigned. Carry out and document interventions to assist patient/community to achieve expected outcomes. Use scientific knowledge, critical thinking, technical skills, and effective interpersonal skills to provide direct patient care as needed.

20% Evaluate and document response to nursing interventions and achievement of outcomes at appropriately determined intervals; as part of a multidisciplinary team, revise plan of care based on evaluative data.

20% Delegate tasks in accordance with specific criteria and state statutes. Review work performed by those to whom it is delegated. Coordinate continuity of care as indicated by patient/family/community needs and resources along health/wellness continuum in collaboration with other members of the health care team. Participate in development of self, peers and others, e.g., MAs, nursing students. Coach other employees and provide feedback to others, as appropriate.

10% Actively participate in process improvement activities to achieve targeted measures of clinical quality, customer satisfaction, and financial performance. Act as patient advocate. Participate and share information from committees, research, and self‑development programs. Communicate and participate appropriately in staff meetings, and collaborate with providers. Manage resources including personnel, supplies and equipment to maintain a safe and efficient environment.

Minimum Qualifications
High School or GED (Required). BLS – Basic Life Support – American Heart Association. LPN – Licensed Practical Nurse – National Council of State Boards of Nursing. Must hold a current license from the State of Ohio. Successfully completed Ohio Board of Nursing approved medication course. Basic Life Support Provider.
