LPN Care Coordinator

Position Summary

The LPN Care Coordinator is accountable for assisting the Director of Nursing/Assistant Director of Nursing and Nurse Manager, RN, in the management and leadership of a specific nursing unit. This position, under the direction of the Director of Nursing and RN Nurse Manager, assists in the evaluation of available resources to assure quality outcomes of care. They also assist in the direction and implementation of the nursing process. This includes assisting the Director of Nursing and RN Nurse Manager with systematic data collection, assisting with the development of the care plan, implementation of the care plan, and revision of the plan. The Care Coordinator, LPN, coordinates the management of the care of residents through a well-defined care delivery system.

Responsibilities

Care Planning and Assessment: Assists in the development and implementation of comprehensive care plans by collecting resident data, contributing to assessments (MDS, RAPS), and collaborating with the interdisciplinary team.
Resident Monitoring and Support: Observes and reports changes in resident conditions to RNs, participates in care conferences, and ensures care plans are updated to reflect current needs and physician orders.
Nursing Care and Leadership: Provides nursing care under RN direction, supervises and supports LPNs and nursing assistants, manages staffing needs, and ensures safe, competent care delivery.
Documentation and Communication: Maintains accurate and timely documentation of resident assessments, incidents, care plans, and medications, and communicates care needs clearly to staff.
Compliance and Quality Standards: Adheres to nursing policies, resident rights, and state/federal regulations; supports quality improvement initiatives and participates in audits and inspections.
Professional Development and Workplace Culture: Maintains licensure, attends ongoing education and in-services, and fosters a positive, inclusive, and customer service–oriented work environment.

Qualifications

Licensure and Certification Requirements: Must hold a current LPN license in Minnesota and complete CPR/emergency care certification; must meet employment eligibility per MN Department of Human Services background check.
Communication and Functional Ability: Must be able to effectively communicate in both verbal and written forms and perform all essential job functions with or without reasonable accommodation.
Preferred Experience and Knowledge: Experience in healthcare settings like skilled nursing or assisted living is preferred, along with familiarity with Medicare coverage and eligibility.
Core Skills and Attributes: Requires strong multitasking, adaptability, emotional intelligence, active listening, and the ability to work effectively in fast-paced, diverse, and evolving environments.
