This position is for local FL residents only and requires in-person, face-to-face training for 90 days at the Winter Garden, FL location before going remote.
Job Summary
Responsible for ensuring that all care team buckets, individual assigned items, abnormal PAP, labs, diagnostics, telephone encounters (TE), and documents within the electronic health record (EHR) are acted upon, tracked, communicated, and documented within the identified time frame. This is a full-time remote position.

Primary Responsibilities and Specific Duties

Familiar with navigating daily tasks and workflows within an electronic health record (EHR) including creating and updating electronic documentation, results review, order entry, and clinical decision support.
Participates in patient care coordination, one-on-one communication, email, patient contact, and documentation of any services accessed and/or health care related concerns/needs that have arisen since last visit.
Accurately processes all incoming patient information to ensure appropriate coordination and continuity of patient care.
Routine clinical duties:
Perform professional manner, maintain strong interpersonal, listening, and communication skills to exceed customer service expectations.
Collect and gather patient information pertinent to encounter; document all communication to patients and clinicians.
Review documentation in a timely manner and within CHC guidelines for documentation.
Communicate with clinician frequently regarding patient care.
Fax orders or prescriptions to hospitals, pharmacies, or other vendors.
Assist clinicians with administrative paperwork.
Schedule patient appointments as needed.
Communicate to assigned supervisor/provider or lead all cases needing a higher level of intervention in a timely fashion.
Maintain, respond, and utilize working relationships with and knowledge of referral sources, community resources, and collaborate with other departments.
Answers the telephone promptly in a courteous and professional manner.
Obtains and enters accurate demographic information into CHC electronic health record (address, telephone number, insurance information, etc.).
Schedules patient appointments according to scheduling protocols/guidelines.
Reviews, updates and confirms appointment information including date, time, location, provider name, reason for visit with patient.
Answers questions and offers other information, as requested or needed.
Performs outbound calls to cancel or reschedule patient appointments and/or provide additional information as needed.
Performs other duties as assigned.
Qualifications
Education
High School or equivalent; completion of accredited licensed practical nursing (LPN) course required.
Experience
Minimum of 1 year experience as a licensed LPN in health care, managed care, physician practice, or clinical setting preferred.
Familiarity with navigating daily tasks and workflows within an electronic health record (EHR) including creating and updating electronic documentation, results review, order entry, and clinical decision support.
Ability to demonstrate organizational skills and attention to detail and possess a good working knowledge of electronic medical records.
Certification/Licensure
Must possess and maintain a current/valid Florida LPN license.
Current/valid BLS certification or must be obtained within the first 30 days of employment.
