Position Summary
The Department of Psychiatry and Behavioral Sciences at Morehouse School of Medicine is seeking an LPN, MSW or LMSW to support the clinicians at the Morehouse Healthcare Youth Mental Health Clinic. The successful candidate should be community oriented with a demonstrated interest in the advancement of mental health equity through clinical care. This is an on-site role. The Care Coordinator is responsible for coordinating and facilitating the entire support process for each of the youth/families served by maintaining accurate records of meetings, timely requisitions and reporting of service requests, resources, and ongoing communication with all team members. This role is crucial in supporting the implementation of timely individualized goal-oriented action plans for youth and useful safety plans for crisis situations.

Minimum Qualifications

Licensed as a Licensed Practical Nurse (LPN), Master Licensed Social Worker, or a Master Social Worker.

Excellent verbal and written communication skills.

Excellent organizational skills and attention to detail.

Excellent time management skills with proven ability to meet deadlines.

Proficiency in Microsoft 365 Suite, or similar software with ability to learn new or updated software.

Strong strategic, analytical, and problem-solving skills.

Two (2) years of relevant experience coordinating resources and services for youth and families.

Preferred Qualifications

Experience in coordinating care for youth and families in behavioral health settings.

Responsibilities / Job Duties

Coordinate ongoing meetings with youth, families, partner agencies and community members, and school systems to identify strengths and explore needs of the team, youth and families served.

Organize and prepare for meetings, including drafting and developing action and safety plans.

Identify and develop individualized resources to meet the unique needs of the youth and families served.

Assist clinicians in coordinating care, supporting parents in referral follow-up, identifying community resources, and other assigned duties.
