Overview
For this position we are seeking a Licensed Behavioral Health Professional (LMSW, LCSW, LPC, LMFT) who lives in Kentucky and must be licensed for the state of Kentucky.

This position will support our Kentucky Medicaid population and will work with Re-Entry Members. The role has a caseload and involves developing relationships with external partners and State Prison staff. Experience with adolescent population is preferred.

This role is primarily remote, but the candidate must be able to go into the field when needed, including travel to prison facilities (mileage reimbursed).

Schedule: Monday through Friday, 8:00 AM to 5:00 PM EST (no weekends or holidays).

Molina Healthcare Services (HCS) works with members, providers and multidisciplinary teams to assess, facilitate, plan and coordinate integrated care across behavioral health and long-term care, for members with high-need potential. HCS staff ensure progress toward outcomes with quality, medically appropriate and cost-effective care based on illness severity and site of service.

Responsibilities

Completes clinical assessments of members per regulated timelines and determines eligibility for case management based on clinical judgment, changes in health or psychosocial wellness, and assessment triggers.

Develops and implements a case management plan in collaboration with the member, caregiver, physician and/or other healthcare professionals and the member's support network to address needs and goals.

Conducts telephonic, face-to-face or home visits as required.

Performs ongoing monitoring of the care plan to evaluate effectiveness, document interventions and goal achievement, and adjust as needed.

Maintains ongoing member caseload for regular outreach and management.

Promotes integration of services for members, including behavioral health and long-term services and supports, to enhance continuity of care.

Facilitates interdisciplinary care team meetings and informal ICT collaboration.

Uses motivational interviewing and Molina clinical guideposts to educate, support and motivate change during member contacts.

Assesses barriers to care and coordinates care to address member concerns.

Collaborates with RN case managers/supervisors as needed.

Case managers in Behavioral Health and Social Science fields may provide consultation and resources to peers as needed.

Local travel up to 20% may be required, depending on member needs and state regulations, or if located within Molina’s Central Programs unit.

Qualifications

Required Education: Any of the following: completion of an accredited LVN/LPN program or Bachelor's or Master's degree (preferably in social science, psychology, gerontology, public health, social work, or related).

Required Experience: 1-3 years in case management, disease management, managed care, or medical/behavioral health settings.

Required License/Certification: Active, unrestricted license in good standing for the state; valid driver's license with a good driving record and reliable transportation.

State Specific Requirements: Kentucky-specific requirements as applicable.

Preferred Experience: 3-5 years in case management, disease management, managed care, or medical/behavioral health settings.

Preferred Licenses/Certifications: LCSW, APSW, CCM, CHEP, LPC/LPCC, LMFT or other related licenses/certifications as listed.

Pay Range: $22.80 - $46.81 hourly.
