Position Overview
Location: Los Angeles, CA 90004. Salary: $24.78 – $30.20 per hour, 24 pay periods per year. Full‑time, non‑exempt, driving required. This position is part of PATH’s new Scattered‑Site Program, Law Enforcement Assisted Diversion (LEAD), and will be based in our regional headquarters while providing field‑based services across LA County.

Responsibilities

Oversee program contracts and ensure successful implementation, including coordination of participant services and achievement of performance measures.

Maintain compliance with contracts and partnership agreements in collaboration with the Quality Assurance Department.

Serve as contact representative for program matters.

Provide administrative oversight of the team, including coverage for the program manager when necessary.

Complete critical incident reports per PATH policies and contract requirements.

Conduct intake and individualized needs assessments for all clients and develop Individualized Service Plans (ISP) addressing housing, income, medical, mental health, substance use, financial, vocational, and social support needs.

Preferred Qualifications

Three years of experience in healthcare, case management, homeless services, or working with vulnerable populations.

Minimum Qualifications

One year of experience in healthcare, case management, homeless services, or working with vulnerable populations.

Mandatory Requirements

Employment eligibility verification.

Ability to obtain CPR/First Aid training.

Successful completion of Tuberculosis Test, Background Screening, and Drug Test.

Driving

Reliable transportation and valid driver’s license.

Proof of insurance and ownership for personal vehicle used during work duties.

Ability to qualify for PATH's insurance coverage.

Employment Eligibility & California Work Expectations
Applicants must be authorized to work in the U.S. PATH cannot sponsor visas. All regular and customary work must be performed within California. PATH does not offer fully remote positions.
