Overview Position: Intensive Case Manager in the Treatment Works (TW 2.0) Program, within the Outpatient Services Division. TW 2.0 provides children’s therapy and High-Fidelity Wraparound (HFW) services for youth and families, designed for children who have experienced trauma, including loss and grief, depression, anxiety, abuse histories, and addictions. Services can be provided in the office or in the community. TW 2.0 supports youth or families, addressing mild to moderate behavioral or emotional needs stemming from trauma, such as school behavior difficulties, adoption-related issues, thoughts of suicide or self-harm, abuse histories, and addictions. All TW 2.0 programming uses the High Fidelity Wraparound model and evidence-based or research-supported practices to help families with problem solving, behavior strategies, and anger management, among other therapeutic interventions.

Responsibilities Provide Intensive Case Management services in the TW 2.0 program. Utilize High Fidelity Wraparound methods and family-centered, solution-focused approaches to support children and families. Collaborate with youth, families, and community resources to address trauma-related needs and improve behavioral and emotional outcomes. Deliver services in both office and community settings and document services in accordance with program requirements.
Education and Qualifications At least two years’ experience in human services or child welfare programs with a Bachelor’s degree in social work or a related field.
Must possess a reliable vehicle for field work, maintain a safe driving record, have automobile insurance coverage as required, and undergo an annual vehicle safety inspection if transporting clients.
Must possess or be eligible for targeted case management certification and Medicaid billing provider number. Must become a Provisional Certified Behavioral Health Case Manager within the first 90 days, and fully certified within one year of hire.
Must become certified as a Wraparound Facilitator in High Fidelity Wraparound within 3 months of employment. Preference given to those already certified.
Ability to acquire knowledge of community resources and experience working with families and children, including school-aged children with histories of suicidal thoughts or actions.
Pay and Benefits Pay Rate: $33.73/hour ($70,169 annual).
