Position Summary
This is a professional position providing Intensive Case Management services in the Treatment Works (TW 1.0) Program (COP) under the Outpatient Services Division. The TW 1.0 program provides an intensive wraparound approach to engaging and working with youth who have been identified as displaying violence or are identified as at risk of violence, as part of the Pinellas County School system. TW 1.0 ensures that youth and families are stabilized and prepared to remain in or reenter their educational environment. The TW 1.0 program ensures that youth and families have time sensitive access to mental health, substance use treatment and supportive services focused on strengthening youth and families and proactively supports those youth and families at risk of expulsion from school or law enforcement involvement. All TW 1.0 programming utilizes a solution-focused, family centered approach and evidenced based or research supported practices to assist families in meeting needs and goals while preventing further incidents at school or with law enforcement. Treatment Works 1.0 will work with all members of the family with a team-based approach in a supportive, empowering and respectful manner to assist the family with creating safe and stable coping skills and parenting practices to facilitate long-term behavior change that will ensure that youth are safe in their school and home.
The ICM is responsible for ensuring an annual client facing client care number of 1410 hours annually. Achieving the minimum required weekly, monthly and annual client facing client care hours is an essential function of this position.
Directions for Living is an essential services provider. All Directions for Living employees must be willing and able to provide vital support and essential services to the people that we serve in the event of an emergency, pandemic or other disaster situation.
Qualifications
EDUCATION EXPERIENCE/REQUIREMENTS:
Must hold at least a Bachelor's Degree in Social Work or related area of study from an accredited university; or a Master's Degree in Social Work or related area of study with a minimum of 2 years of experience working with at-risk populations to include families experiencing substance misuse, domestic violence, mental health.
Must possess a reliable vehicle to drive to do field work and maintain a safe driving record, maintain specific automobile insurance coverage as required by the organization and undergo an annual vehicle safety inspection if transporting clients.
Must possess or be eligible for targeted case management certification and Medicaid billing provider number.
Must become certified as a Wraparound Facilitator in high fidelity Wraparound within 3 months of employment. Preference will be given to individuals certified as a Wraparound Facilitator in high fidelity Wraparound.
Experience with school aged children with histories of violence or threats of violence, required.
