Overview
Intensive Case Manager (Nashville) at Inspiritus. Classification: Non-Exempt. Salary: $40,000 - $45,000. Reports to: Client Services Manager. Date: 1/14/25.
Responsibilities
Provide intensive case management to address needs that extend beyond the initial resettlement period, focusing on medical, educational, housing, and clients’ special needs.
Develop individualized, holistic service plans based on client needs.
Conduct RHS-15 mental health screening for all ICM clients and make timely referrals to mental health care providers.
Network with other social service providers, including mainstream providers and those that serve refugees exclusively.
Develop and maintain a database of client services to supplement Inspiritus offerings.
Develop and facilitate periodic community support groups for Inspiritus clients to promote community connections and healthy coping strategies during integration.
Arrange client appointments and share schedules with clients and others assisting clients (e.g., case managers, interpreters, volunteers).
Provide orientation about health-related issues, Medicaid education, and identifying physicians with language-appropriate services.
Refer clients to one-on-one volunteer mentoring/First Friend program in coordination with the Community Engagement Specialist.
Assist clients in resolving emergency issues and keep the Client Services Manager apprised of difficult cases.
Serve as interpreter when language skills are applicable for case services and maintain professional boundaries.
Foster respectful partnerships with clients, co-workers, and employers; participate in Inspiritus functions and public relations activities.
Participate in in-service training, conferences, and PQI activities; comply with regulations, policies, work procedures, and applicable laws.

Work Environment
Requires travel by automobile; 50% travel locally. Will make home visits to refugee families and transport clients to appointments as needed. May work outside regular hours to accommodate client needs. Some duties may be performed at an out-posted location as assigned.
Physical Demands
Regularly able to talk and hear; able to stand, walk, sit, and manipulate objects. Able to operate a personal vehicle. May lift up to 10 pounds. Vision requirements include close vision and the ability to adjust focus.
Position Type and Hours
Full-time. Monday through Friday, 8:30 a.m. to 5:00 p.m. Travel up to 50% locally.
Education and Experience
Required:
Bachelor’s degree with concentration in a relevant field (social work, counseling, public health, or psychology)
Two years of direct work experience in a culturally diverse environment, or equivalent combination of education and experience
Ability to organize multiple client-related activities; excellent English communication skills; ability to balance client and employer needs
Excellent computer skills (Word, Excel, Outlook); culturally competent communication; sensitivity to the service population’s characteristics
Must own a personal multi-passenger vehicle, hold a valid driver’s license for Georgia/Tennessee/Alabama, and have personal auto insurance and a good driving record
Preferred:
Master’s degree in a relevant field and 2+ years of applicable human services experience
Experience in case management, health advocacy, community outreach, and partnership development
Experience in maternal/child health, reproductive health, medical/insurance systems, or health education
Knowledge of immigration and/or refugee issues
Ability to read, write, and speak a relevant foreign language
