Overview Inpatient Case Manager, MSW role with Sentara Regional Resource Pool located in Hampton Roads. Multiple shifts available. The position requires working at four hospitals in the Southside or Western Tidewater regions, with location and unit assigned based on greatest need. Regions include Southside, Peninsula, and Western Tidewater; must be within 75 miles and 90 minutes of all four hospitals in the assigned region.
Operational hours: 8:00 AM – 4:30 PM, 7 days per week. Flex hours: at least 48 hours of prescheduled time per calendar month (with weekend availability required for 16 hours). Team members may work in all units within the case management specialty and may travel to multiple hospitals within the assigned region.
Responsibilities
Provide medical social work services to hospital patients, assessing social, psychological, cultural, environmental, and financial needs and disposition planning.
Collaborate with patients, families, healthcare teams, and community organizations; advocate to identify needs and resources in-hospital and in the community.
Develop and evaluate transition plans considering diagnosis, prognosis, services, goals, provider options, and available benefits across the continuum of care.
Work in hospitals only within the designated region (Southside or Western Tidewater) and support 4 hospitals in the region.
Education, Certification & Licensure
Master's degree in Social Work.
Certification in Case Management (CCM, CMAC or ACM-SW) required within one year of eligibility.
BLS certification required within 90 days of hire.
Experience & Requirements
One year of related experience.
Must be within 75 miles and 90 minutes of all four hospitals in the assigned region.
