Responsibilities
The essential function is to work in partnership with Dual Diagnosed people receiving services based upon specific objectives identified in the treatment plan. The approach is strengths based using the IDDT model. Staff training in the IDDT model will be conducted. The essential function of the case manager is to work in partnership with people receiving services toward individual goal attainment based upon specific objectives identified in the treatment plan. The approach is strengths based, and most of the service is provided in the community. Case managers must be able to transport people served, have a reliable vehicle, a valid Kansas driver’s license and necessary insurance coverage. Must be able to pass a KBI check and must not be listed on any registries. Each case manager is responsible for an assigned caseload.
Job Requirements
a person with a Bachelor’s Degree in social/human service field
a person with an Associate of Arts degree in Mental Health Technology with experience
a Licensed Mental Health Technician with experience
an individual equally qualified by work experience or a combination of work experience and education, with one year of experience substituting for one year of education
Experience with persons with severe and persistent mental illness and/or chemical dependency is preferred
Has the ability to teach, train, and model professional skills and behavior related to IDDT model
A valid Kansas driver’s license and access to reliable personal transportation and necessary insurance is required
Salary Range
$23.00 - $26.97
Hours
Monday – Friday 8am – 5pm
