Responsibilities
Provide case management services in the community to individuals with intellectual & developmental disabilities and their families.
Facilitate linking, advocating, coordinating, and monitoring services to assist consumers in gaining access to health services, financial assistance, employment, education, social services, and natural supports.
Assess needs, develop, and monitor Person Centered Plans and documents treatment progress.
Qualifications
Bachelor in Social Work or Master’s Degree in an appropriate mental health discipline.
QIDP, one year’s experience providing treatment to individuals with Intellectual and Developmental Disabilities.
Must have reliable means of transportation for home visits and maintain a Safe Driving Permit.
