Overview Riverwood Center provides a range of personalized services to individuals with behavioral health, intellectual & developmental disabilities, and substance use disorders. Located in southwest Michigan, the organization has staff at seven sites across Berrien County and is accredited by CARF. Riverwood Center is a member of Southwest Michigan Behavioral Health, the Michigan Association of Community Mental Health Boards, and the National Council for Behavioral Health.

Responsibilities Provides case management services in the community to individuals with intellectual and developmental disabilities and their families
Facilitates linking, advocating, coordinating, and monitoring services; to assist consumers in gaining access to health services, financial assistance, employment, education, social services, and natural supports
Assesses needs, develops, and monitors Person Centered Plans and documents treatment
Qualifications Bachelor in Social Work or Master's Degree in an appropriate mental health discipline
QIDP, one years’ experience providing treatment to individuals with Intellectual and Developmental Disabilities
Must have reliable means of transportation for home visits and maintain Safe Driving Permit
