ICMS Case Manager - Pathway Home Project (JR 5269) role at PATH

Pathway Home Project, Path: ICMS Case Manager, is a field-based, direct client services role within PATH’s Intensive Case Management Services (ICMS) program. The Case Manager provides services to individuals experiencing homelessness with chronic medical conditions and co-occurring disorders who are high utilizers of the DHS hospital and outpatient system. The position is based in Los Angeles, CA 90004.

Responsibilities

Provide case management services including independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, and linkage to community providers for substance use, primary and mental health care.

Collaborate with clients to develop individualized treatment plans addressing short- and long-term goals.

Provide ongoing case management support to assess plan progress and adjust goals as necessary.

Provide strengths-based case management and service coordination to help clients obtain and maintain stable housing.

Maintain documentation standards as set forth by the program contract and PATH policies.

What You Bring

Maintain and execute confidential information according to HIPAA standards.

Knowledge or experience with evidence-based case management techniques including critical time intervention, trauma-informed care, harm reduction strategies, crisis intervention techniques and motivational interviewing.

High tolerance and understanding for individuals with urgent and multiple case management and health needs.

Ability to work independently as well as in a team; mature judgment; motivated, self-starting and proactive.

Excellent communication skills, both written and verbal.

Strong prioritization skills and ability to coordinate multiple demands in a high-pressure environment.

Preferred Qualifications

Two (2) years of experience in healthcare, case management, homeless services or working with vulnerable populations or a related field.

Minimum Qualifications

All levels of education and experience are welcome.

Mandatory Requirements

Be able and willing to work flexible hours including evenings or weekends.

Have employment eligibility verification.

Have or be able and willing to obtain CPR/First Aid training.

Provide proof of full COVID-19 vaccination.

Successfully complete: Tuberculosis Test, Background Screening, Drug Test.

Driving
Driving is an essential function of this position. Requirements include:

Reliable transportation and a valid driver’s license.

Proof of insurance and ownership for personal vehicles used during work duties.

Ability to qualify for PATH's insurance coverage.

California-based Work Expectations
PATH is a California employer; regular work must be performed within the state. Some roles may include designated worksites or approved off-site locations, but this is not fully remote work. Schedules are determined by role responsibilities and may change.
