About Our Program
Easterseals, a national nonprofit, has supported individuals with special needs for over 50 years. Its services for individuals with mental health conditions provide daily living support and community-based programs that teach valuable life skills through instruction and real world experiences.
What You’ll Do
Through community outreach within Hunterdon County, NJ, provide ongoing support to consumers to help them function independently and access mental health services.
Conduct home visits to engage with consumers in their environments to reduce risks of hospitalization and promote functional integration into the community.
Assist consumers in accessing needed services and ensure transportation to appointments.
Develop individualized service plans and ensure the consumer’s engagement with community mental health and non-mental health services.
Assess the need for crisis intervention and assist in psychiatric emergency situations.
Encourage wellness and recovery while maintaining individual dignity and respect.
Work with multi-disciplinary teams to develop discharge plans and ensure continuity of care.
Who You Are
Bachelor’s Degree in Social Work, Psychology, Sociology, Human Services or a related field from an accredited college or university required.
At least 2 years of case management experience with community-based mental health services is preferred.
Strong computer skills (Microsoft Office Suite including Word & Excel).
Valid driver’s license and acceptable driving record.
Excellent communication, planning, and organizational skills.
A compassionate, flexible, and client-focused approach to care.
