Responsibilities The essential function of this case manager is to work in partnership with consumers toward individual goal attainment based upon specific objectives identified in the treatment plan. The approach is strengths based, and most services are provided in the community. Case managers must be able to transport consumers, have a reliable vehicle, a valid Kansas driver’s license and necessary insurance coverage. Must be able to pass a KBI check and must not be listed on any registries. Each case manager is responsible for an assigned caseload. This case manager will also bridge the gap in maintaining and engaging services while utilizing community services (in hospitals – KU Med St Francis Campus, Stormont Vail Hospital, Topeka ER as well as in jail).
Requirements This position requires: 1) a person with a bachelor’s degree in social/human service field, 2) a person with an Associate of Arts degree in Mental Health Technology with experience, 3) a Licensed Mental Health Technician with experience, or 4) an individual equally qualified by work experience or a combination of work experience and education, with one year of experience substituting for one year of education. Experience with persons with severe and persistent mental illness and/or chemical dependency is preferred. A valid Kansas driver’s license and access to reliable personal transportation and necessary insurance is required.
Salary Range $23.00 - $26.97
