We are hiring full-time RN Case Managers to join the Hospice Services of Massachusetts team!

This position is an on-the-road role covering portions of Middlesex and Essex Counties. Weekly pay!

Responsibilities

Plan, organize, and direct hospice care as an RN Case Manager. Build from community resources to plan and direct services to meet the needs of individuals and families within their homes and communities.

Supervise hospice aides as assigned.

Complete an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs. Provide a complete physical assessment and history of current and previous illness(es).

Provide professional nursing care by utilizing all elements of the nursing process.

Assess and evaluate patient status by writing and initiating plan of care, regularly re-evaluating patient and family/caregiver needs, and revising the plan of care as necessary.

Initiate the plan of care and make revisions as patient status and needs change.

Use health assessment data to determine nursing diagnosis.

Develop a care plan that establishes goals based on nursing diagnosis and incorporates palliative nursing actions, including the patient and family in the planning process.

Initiate appropriate preventive and rehabilitative nursing procedures. Administer medications and treatments as prescribed by the physician in the plan of care.

Counsel the patient and family in meeting nursing and related needs and provide health care instructions as appropriate per assessment and plan.

Assist the patient with activities of daily living and support self-sufficiency and comfort care as appropriate.

Act as Case Manager when assigned by Clinical Supervisor and coordinate patient care for the assigned caseload.

Qualifications

Graduate of an accredited school of nursing; BSN preferred.

One to two years of recent acute care experience in an institutional setting, and one to two years of recent experience in home care.

Current licensure as an RN in Massachusetts and CPR certification.
