Role Description
This is a full-time role for a Hospice RN Case Manager that will work out of our Sealy office. The territory of patients served by this nurse will primarily be around the communities of Brenham and Bellville. The Hospice RN Case Manager will oversee the day-to-day care of hospice patients, including coordinating plans of care, managing symptoms, providing educational and emotional support to patients and families, and collaborating with interdisciplinary team members. Responsibilities also include nursing assessments, medication management, and ensuring compliance with hospice regulations and protocols.
Qualifications
Excellent communication and interpersonal skills
Ability to work collaboratively with a team
Current RN license in the state of Texas
Prior hospice or palliative care experience is a plus
