About the Role
The Registered Nurse Case Manager (RN Case Manager) will provide direct patient care to hospice patients and support the patient’s family. They will manage an interdisciplinary group (IDG) of Licensed Practical/Vocational Nurses, Certified Nursing Assistants and support staff to enhance hospice care, and will case manage all assigned patient clinical records.
Schedule: Monday - Friday 8:30am-5:00pm

Role Responsibilities Promote and practice the Sage Family of Companies philosophy and mission of providing compassionate, comprehensive, and qualified care to people with a life-limiting illness, serving them with dignity, respect, and integrity.
Assure the highest quality of care is provided and documented in accordance with the Plan of Care (POC).
Perform initial and ongoing assessment of the patient.
Develop and implement the POC as prescribed by the physician, in conjunction with other IDG members.
Educate the patient, family, or other caregivers on needs identified in the POC, including medication administration, dressing changes, oxygen administration, hospice philosophy, and care of the terminally ill patient.
Collaborate with IDG to provide comprehensive physical, psychosocial, educational, emotional, and spiritual services, including bereavement support.
Maintain primary contact and professional management of each patient; act as liaison between physician, patient, family, and hospice team for clinical issues, staffing, and scheduling.
Report any change in patient condition to Attending Physician and Hospice Physician.
Maintain close contact with patient families to provide information, support, and continuity of care.
Maintain communication with facility, group home, and private care staff.
Maintain accurate and updated medical records, including IDG notes, POC, and assessments.
Perform ongoing review of patient records to ensure quality services, standards, and regulatory compliance.
Practice infection control measures and standard precautions.
Conduct routine supervisory visits with team members at least monthly.
Monitor scheduling of all staff.
Maintain knowledge of pain and symptom management in the dying process.
Participate in Quality Assessment and Performance Improvement programs.
May participate in on-call schedule as requested.
Promote Sage Hospice and Palliative Care core values and complete annual training.
Perform other duties as assigned.
Role Requirements Registered Nurse license in the state of practice.
One (1) year of experience in oncology, long-term care, hospice, or home health setting.
Current CPR certification.
Reliable transportation, valid driver’s license, and proof of automobile insurance.
