Hospice Registered Nurse (RN) Case Manager

Job Summary
The Hospice Registered Nurse (RN) Case Manager plans, organizes and directs hospice care. The professional nurse builds on community resources to plan and direct services to meet the needs of individuals and families within their homes and communities.
Responsibilities
Completes an initial, comprehensive assessment of patient and family to determine hospice needs.
Provides professional nursing care by utilizing all elements of the nursing process.
Assesses and evaluates patient’s status by writing and initiating a plan of care, regularly re‑evaluating needs, and revising the plan as necessary.
Uses health assessment data to determine nursing diagnosis and develops a care plan that establishes goals based on the nursing diagnosis and incorporates palliative nursing actions.
Administers medications and treatments as prescribed by the physician.
Provides health education and counseling to the patient and family.
Assists the patient with activities of daily living and facilitates self‑sufficiency and comfort care.
Acts as RN Case Manager when assigned by Clinical Supervisor and coordinates patient care for the assigned caseload.
Participates in on‑call duties as defined by the on‑call policy.
Supervises ancillary personnel and delegates responsibilities when required.
Maintains professional knowledge through continuing education and in‑service classes.
Participates in quality assessment and performance improvement teams and activities.
Communication
Maintains accurate and relevant clinical notes regarding patient condition and care.
Communicates with the physician regarding patient needs and reports changes in condition; obtains physician orders as required.
Communicates with community health related persons to coordinate the care plan.
Teaches patient and family self‑care techniques and provides medication, diet, and other instructions as ordered by the physician.
Provides and maintains a safe environment for the patient.
Works in cooperation with the interdisciplinary group to meet the emotional needs of the patient and family.
Attends interdisciplinary group meetings.
Qualifications
A passion for hospice care and the ability to build relationships with both patients and their family members.
Ability to work efficiently with little direct supervision.
A valid driver’s license and an insured automobile in good working order.
1‑2 years of recent acute care experience in an institutional setting (required). 1‑2 years of recent home care experience (preferred).
Graduation from an accredited school of nursing.
Understands hospice philosophy and is comfortable providing specialized care to the terminally ill.
Displays empathy for the needs of the ill, injured, frail and impaired.
Provides a calm demeanor when in a patient’s home.

Schedule
Monday – Friday; 8:00‑5:00 with some on‑call and weekend rotation.
Flexible scheduling available.
Compensation
$75,000 – $85,000 per year
Mileage reimbursement at $0.58 per mile
Work Location
Mix of facility and home‑based hospice care
Coverage Area
Will be assigned a centralized territory
Productivity
Managing an average caseload of 12‑15 patients
