Responsibilities Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs, including a complete physical assessment and history of current and previous illness(es).
Completes initial evaluations, consents, and admissions for patients.
Coordinates patient care for assigned caseload.
Provides professional nursing care by utilizing all elements of the nursing process.
Assesses and evaluates patient status and writes and initiates plan of care; regularly re-evaluates patient and family/caregiver needs; participates in revising the plan of care as necessary.
Initiates and revises the plan of care as patient status and needs change; uses health assessment data to determine nursing diagnoses.
Develops a care plan with goals based on nursing diagnosis, incorporating palliative nursing actions and including the patient and family in the planning process.
Initiates appropriate preventive and rehabilitative nursing procedures; administers medications and treatments as prescribed by the physician in the plan of care.
Counsels patient and family in meeting nursing and related needs; provides health care instructions as appropriate per assessment and plan.
Assists the patient with activities of daily living and facilitates self-sufficiency and optional comfort care.
Maintains a weekly visit minimum as established by Office Administrator.
Works within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and CNA leadership.
Can work independently day-to-day with activities including phone, email and meetings with healthcare professionals, and an understanding of hospice rules and regulations.
Manages time effectively to handle visits, documentation, and meetings; prioritizes urgent needs and triages multiple concerns.
Demonstrates ability to learn an electronic medical records system.
Extensive travel across the service area to healthcare facilities, assisted living homes, patients’ homes, and home office. Must have reliable transportation, a valid driver’s license, and active auto insurance in accordance with state requirements.
Qualifications Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
Ability to work well within a team and collaborate daily with other healthcare professionals
Ability to work independently and communicate effectively via phone, email, and written methods
Strong time management and organizational skills
Ability to learn and use an electronic medical records system
