Position Job title: Patient Care Manager (RN)
Position Summary Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high-quality care to hospice patients assigned to the team’s area in accordance with physician-prescribed plan of care and all applicable state and federal laws and regulations.
Responsibilities Coordinate and supervise an interdisciplinary team to ensure high-quality hospice care
Ensure compliance with physician plans of care and applicable laws and regulations
Provide leadership and supervision to nursing staff
Qualifications Requires college degree; strongly prefer a Registered Nurse/BSN
2+ years of hospice experience and supervisory experience required
Experience in a Charge Nurse role will be considered to meet supervisory requirement
Knowledgeable of hospice regulations
Proficient computer skills; Microsoft Office experience required
Good communication skills and ability to travel independently
