Eden Hospice is hiring a Nurse Practitioner who will work autonomously but in collaboration with Physicians to provide primary medical care to patients with chronic or life limiting illnesses that require Palliative or Hospice care interventions. (Bonner County, Kootenai County and Spokane County)

Job Summary
Understands and communicates the illness trajectory, prognosis, appropriateness of medical treatments, and palliation and symptom management related to the serious illness and other conditions.
Collects and assesses patient care data; develops and implements a plan of care; manages and evaluates outcome of care in collaboration with the patient’s provider(s) and the Palliative Care Team (PCT).
Performs comprehensive assessment on all patients. Adequately assesses and reassesses symptoms.
Provides follow-up care; determines effectiveness of the treatment plan; reassesses and changes the plan as needed in collaboration with patient’s provider(s).
Educates the patient/family regarding symptom management, disease process and advance care planning choices.
Prescribes and/or recommends medications (see individual state laws) and non-pharmacological therapies and DME based on the patient's diagnoses, health history, and age in consultation with the referring provider.
Makes patient visits to provide skilled nursing care serving as patient advocate, care coordinator, and educator.
Participates in PCT meetings/IDG/case conferences as appropriate.
Coordinates with social workers, case managers, and applicable ancillary staff regarding care, treatment, and services.
In collaboration with patient’s provider(s), refers patients to other healthcare services as indicated.
Reports significant changes in the patient’s condition.
Prepares and maintains accurate patient records. Documentation meets all standards and policies. Documentation includes all findings, interventions, and results.
Supports and maintains a culture of safety and quality.
Complies with applicable laws and regulations with respect to Delegation Protocol Agreements, prescriptive authority, and NP scope of practice.
Provides reports to referring provider upon request.
Treats patients and families with respect and dignity, supporting cultural practices, as long as practices do not harm others.
Works collaboratively with Billing Department in verification of coverage or payment.
Participates in Agency’s other mandated activities (e.g. in-services, QAPI committee, staff meetings etc.).
Participates in on-call as assigned.
Other duties as assigned.
Education/Experience Requirements
1. Understands regulations/standards applicable to Palliative Care.
Thorough knowledge and understanding of the philosophy and functions of Palliative Care services.
Demonstrates knowledge of the seriously ill patient and symptom/pain control measures.
Exhibits genuine interest in and compassion for patients and families dealing with serious illness.
Ability to be flexible, organized, and function under stressful situations
Able to communicate effectively in English, both verbally and in writing.
Excellent interpersonal skills.
Excellent writing and presentation skills.
Knowledge of general modalities and scope of practice within the state of Agency operation.
Candidate should be self-directed and can work in the field with minimum supervision.
A valid driver’s license, reliable auto, and current auto insurance.
Basic computer knowledge.
Current Registered Nurse License in the state of employment.
Master’s Degree from an accredited Nurse Practitioner's Program.
National Certification as a Nurse Practitioner.
Current BCLS Certification.
A minimum of one (1) year Nurse Practitioner experience in clinical nursing care of geriatrics, adult/family practice, or oncology preferred.
Advance certification in hospice and palliative nursing care (ACHPN) preferred; required within 30-months of hire.
Previous Palliative Care, Home Health or Hospice experience preferred
