What You’ll Do
Deliver hospice-level nursing care, including routine and emergency "face to face" recertification visits for patients in their third benefit period or beyond.
Record observations, treatments, and other pertinent information.
Communicate findings to Medical Director, Attending Physician, branch leadership, and IDG to address patient needs.
Provide physical and emotional care to support patients and families during crisis.
Triage consults by patient acuity and perform bedside palliative care with patient/family education.
Prevent unwanted hospitalizations by meeting patient and family needs in the home.
Maintain accurate patient records, charts, and documents to support sound medical practice and reimbursement.
Ensure compliance with applicable laws, regulations, and collaborative agreements.
Complete accurate billing and coding activities for all encounters in accordance with CMS guidelines.
Assist in the development of clinical practice guidelines and quality palliative care standards.
Collaborate with Medical Director, attending physician, branch leadership, and IDG.
Perform other duties and projects as assigned.
Qualifications
Master’s degree in nursing and certification through a national Nurse Practitioner licensing board.
1+ year of clinical nursing experience.
1+ year post’graduation clinical nursing experience in the hospice and palliative care field (preferred).
Prior experience in Family Medicine, Geriatrics and psychiatry preferred.
Current licensure in the state of practice as a Nurse Practitioner and maintenance of licensure in accordance with applicable laws and regulations.
Maintain active DEA certification if state permits prescription of controlled substances by APPs.
50% travel required.
Ability to sit, stand, bend, lift and move intermittently and lift 50-100 lbs.
Valid driver’s license and reliable transportation.
