Overview Join to apply for the Hospice Nurse Practitioner role at Athena Health Care Systems. This position assists with on-call admissions visits. Candidate must be willing to travel. $100 per visit. Must be available within 48 hours to perform face-to-face visits.
Duties & Responsibilities Provides medical care, palliates, and manages terminal illness in compliance with the state Nurse Practice Act under the supervision of a collaborating physician.
Completes history and physical examinations by evaluating the patient's medical condition and health history, ordering and interpreting results from lab and diagnostic tests, diagnosing health conditions, and documenting findings in the patient’s medical records.
Manages health conditions and terminal illnesses by developing plans of care, prescribing medications or treatments, obtaining consultations or making referrals, and coordinating hospice care services.
Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification for hospice care.
Reviews clinical information and coordinates patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners.
Completes, maintains, and submits accurate clinical notes, physical examination notes, assessment visit notes, medical orders, collaboration notes, and other documentation in the medical record.
Ensures compliance with Medicare conditions of participation and other state regulations governing the provision of healthcare.
Leverages community resources to plan and direct services that meet the needs of individuals and families.
Experience & Qualifications Master’s degree with a minimum of one (1) year of experience as a nurse practitioner; hospice or palliative care experience preferred.
CPR certification.
DEA license.
Accreditation from an approved certifying body for advanced practice nursing.
Preferred certification in a specialist area such as hospice or pediatrics.
