We are seeking a Hospice RN Case Manager to provide care in Milwaukee County.

RN Case Manager Responsibilities

Make the initial nursing evaluation to determine eligibility for hospice services within 24 hours of referral.

Identify the patient's/family's physical, psychosocial, and environmental needs and re-assess no less than every 15 days.

Initiate and coordinate the plan of care while maintaining accurate and relevant clinical notes.

Document problems, appropriate goals, interventions, and patient/family response to hospice care.

Collaborate with the patient/family, attending physician, and other members of the IDG in providing daily patient and family care.

Experience Requirements

Two years of clinical experience.

License Requirements

Current Wisconsin RN licensure and two years of clinical experience.

Current driver's license, valid vehicle insurance, and access to a dependable vehicle.

Position Details

Location: Milwaukee County

Schedule: Full-Time, 1st shift Monday-Friday. Minimal on-call required.

Compensation: Negotiable based on experience.

Sign-On Bonus: Up to $7,500 available, depending on location and experience.
