Home Health RN Case Manager ~ MarshalTown Area
Join Iowa Home Care as a Registered Nurse Case Manager. In this role you will provide compassionate, patient‑centered care directly in clients' homes, oversee and coordinate all aspects of patient care, develop individualized care plans, and collaborate with an interdisciplinary team to maintain high standards of health and wellness.
Responsibilities
Patient Assessment & Care Planning: Conduct initial and ongoing assessments, determine home care needs, develop and update individualized care plans.
Direct Patient Care: Administer medications and treatments as prescribed, provide skilled nursing care per the State Nurse Practice Act, and implement preventive and rehabilitative procedures.
Care Coordination: Coordinate care across services, collaborate with physicians and community health providers to optimize care plans and outcomes.
Documentation & Communication: Maintain accurate clinical notes, communicate changes in patient status to physicians, and obtain necessary orders.
Patient & Family Education: Provide tailored health instructions, involve families in planning for support.
Supervision of Support Staff: Instruct, supervise, and evaluate home health aides at least bi‑weekly to ensure quality care delivery.

Qualifications
Graduate of an accredited school of nursing; 1–2 years of recent acute care experience in an institutional setting.
Current RN licensure and CPR certification.
Excellent observation, verbal, written communication, problem‑solving, basic math, nursing skills per competency checklist.
Licensed driver with a vehicle in good working order and insured per organization requirements.
