Responsibilities Lead the health care team in establishing and maintaining an effective plan of care for medically complex patients.
Develop and maintain relationships with patients and families to support optimal health outcomes.
Collaborate with physicians and other team members to assess, plan, implement, and evaluate patient care.
Demonstrate relationship-based care with self, colleagues, patients, and families.
Engage in clinical assessment, planning, prioritizing, coordinating, and implementing care.
Act as an empowered leader participating in shared governance and ongoing professional development.
Commit to quality achievement through adherence to documentation guidelines, outcomes data, and performance improvement activities.
Apply innovative practices and evidence-based care, including the use of technologies that support patient care.

Qualifications Education: Graduate of an accredited nursing program; Bachelor of Science in Nursing required within 5 years of hire.
Experience: 1 year of relevant experience.
Licensure: Licensed to practice as a Registered Nurse in the Commonwealth of Virginia; valid Virginia Driver’s License.
Certifications: American Heart Association (AHA) Health Care Provider BLS certification.
Physical/other requirements: Prolonged sitting, frequent standing, travel, bending/stooping; ability to lift/push/pull 20-50 lbs; proficient communication, auditory and visual skills; attention to detail.

Compensation Position Compensation Range: $82,992.00 - $110,240.00 annual.
