Position Summary
POSITION SUMMARY – Home Health Case Manager RN
POSITION DUTIES – The PACE Home Health Case Manager RN promotes and maintains the health of participants in the community through teaching, counseling, and coordination of appropriate preventive and rehabilitative services with the PACE interdisciplinary team. Responsible for assessment of health needs and oversight of the plan of care, initial implementation of nursing care plans and providing nursing care, as well as periodic re-evaluation of individual and family nursing needs. This role provides technical, organizational, and interpersonal skills necessary to coordinate the scheduling, communication, and encounter documentation of all aspects of the plan efficiently and effectively for home care. Oversees and updates the CNA plan of care.
Minimum Required Qualifications
Unrestricted CO/compact RN license
Valid driver’s license, reliable transportation and auto insurance coverage
Cell phone and laptop provided for documentation
Minimum of one year acute care experience in medical/surgical, ICU, ER or other acute or home health setting experience
Strong communication skills and a desire to work in a team environment
LENGTH OF ASSIGNMENT – 13 weeks
SHIFT / HOURS PER WEEK – Monday-Friday, 8a-5p
START DATE – ASAP
