Job Summary
Responsible for the development and implementation of home‑care services for Florida PACE Center participants, including the coordination of all Durable Medical Equipment and nursing home care.
Essential Job Functions
Assess the home‑care needs of participants, identifying and developing specific plans of care.
Assist in safety evaluations for prospective participants prior to enrollment, documenting and uploading findings in EMR.
Coordinate the implementation of all home‑care services—including nursing, physical therapy, occupational therapy, speech therapy, and personal care—to ensure quality services meet client needs.
Coordinate and authorize all Durable Medical Equipment, consumables, and frozen meals.
Coordinate and supervise all contracted nursing care for FPC participants in nursing homes/boarding homes according to program standards, interacting with other facility staff.
Assess program home‑care services system, making recommendations and offering solutions to revise or modify the system to better meet participants’ needs.
Actively participate in multidisciplinary team meetings.
Perform compliance monitoring of contracted home‑care agencies and personnel, including all ALF audits for quality purposes.
Participate in department meetings and in‑services as necessary.
Prepare and coordinate Health Assessment Form 1823 for ALF’s contracted by Florida PACE.
Education
Graduate of an accredited school of practical nursing (preferred) or higher level of education.
Experience
One to three years of experience working with frail elderly populations and home‑care (or a combination of experience and education).
Licenses / Certifications
Current LPN license issued by the Florida State Board of Nursing (preferred).
Current BLS certification.
Valid Florida driver’s license and proof of insurance.
Abilities Required
Sensitivity when working with impaired elderly clients.
Good interpersonal skills and ability to work with staff, clients, and providers of diverse ethnic backgrounds.
Organized with good judgment.
Excellent communication skills in English.
Knowledge of Spanish may be required.
Florida driver’s license, insurance, and access to a vehicle for field visits.
Ability to demonstrate good judgment regarding personal and client safety in the role of a community service provider.
Willingness to conduct in‑home visits anywhere in Dade and Broward counties.
