About the Role
The Home Care Coordinator will serve as the primary administrative contact and service coordinator for home‑care clients across three communities in Philadelphia, Downingtown, and West Grove. Under the general guidance of the Home Care Director, the Coordinator will manage scheduling, client care, staff supervision, and general office duties.

Responsibilities

Scheduling

Create, implement, and maintain both care partner and client schedules.

Match staff with clients based on needs, care plans, skills, experience, availability, and personality.

Ensure staffing levels meet client care plans and communicate changes to partners and clients.

Provide caregiving services when needed to maintain schedule in accordance with client care plans.

Assessments & New Client Service

Coordinate all assessments and ongoing services to meet client needs.

Administrative

Maintain accurate documentation in electronic medical records, scheduling platforms, billing, payroll, client care, staff performance, and attendance.

Assist with preparing client and staffing information for Department of Health licensing renewal surveys.

Staffing

Interview, onboard, orient, and train new staff members.

Communicate staffing needs to the Home Care Director to ensure client needs are met and promote operational growth.

Management of Staff

Recognize and reward good job performance and promote care partner recognition.

Conduct performance reviews, disciplinary plans, and address employee and client satisfaction issues.

Escalate disciplinary action to the Home Care Director.

Caregiver Training

Orient care partners to clients, care plans, and needs.

Assist with quarterly staff training meetings and monitor completion of mandated corporate and state trainings.

Client Assessments and Care Planning

Respond to all client inquiries for services.

Work with internal referral sources to assess needs and establish care for new clients.

Create, implement, and maintain care plans and schedules to meet client needs.

Case Management

Communicate regularly with client, care partners, family, and interdisciplinary team regarding progress and needs.

Monitor status and adjust care plans as needed.

Collaboration

Participate as a member of the Simpson Communities Interdisciplinary Teams and Management Teams.

Maintain positive relationships with coworkers, clients, vendors, and referral sources.

Communicate staff updates and client changes to the Director of Home Care.

Office Management

Promote a positive work culture aligned with Simpson’s values of compassion, integrity, and respect.

Maintain an efficient, organized, and welcoming office for staff and guests.

Answer calls, take accurate referral/service requests, and maintain office supplies.

Physical Requirements
Must be able to stand, sit, walk, maintain balance, kneel, crouch, grasp, bend, pull, and reach for extended periods throughout the working day, perform repetitive motions, and lift or push/pull a minimum of 50 lbs.

Equipment and Tools
General office equipment: computers, laptops, tablets, telephone, smart cell phone, calculator, copiers, fax, communication and pager equipment, and projectors.

Education or Degree Required
High School Diploma or GED.

Licenses, Certifications, Registrations

Valid Driver’s License, proof of valid auto insurance and registration, a clean PA DMV driving record, reliable personal transportation, and Pennsylvania Child Abuse History Clearance.

Qualifications and Related Experience

Home Care field experience (minimum 3 years).

Care Coordinator and/or scheduling experience in the health care field (minimum 3 years).

Leadership experience (minimum 1 year).

Proficient in computer skills including Microsoft Office Suite.

Be at least 21 years of age.

Must be able to work independently while adjusting daily job priorities to satisfy changing client needs.

Travel between resource center and communities required.
